To find the best health care coverage for you, consider your health care needs, your preferred providers, and your budget. Health plan types
- Keystone HMO Proactive plans: Our most popular plans, which focus on lower premiums and options to save on care with a tiered network. You still have access to the full provider network.
- Keystone HMO plans: With these plans, you would choose a primary care physician (PCP), who will coordinate your care.
- Personal Choice PPO plans: Offering the largest provider network, these plans also give you the freedom to see providers both in- and out-of-network without referrals.
- Personal Choice EPO plans: These plans offer in-network only coverage (except for emergencies), but your network includes the national BlueCard PPO network. You do not need to select a PCP or get referrals to see specialists.
- Personal Choice EPO Reserve plans: You’ll have coverage under an EPO plan, coupled with a tax-advantaged health savings account (HSA) for any qualified medical expenses.
- Personal Choice EPO Catastrophic plan: This health plan is for individuals younger than 30 and those with an extreme financial hardship.
