¹The Part B Premium Giveback is set up by Medicare and administered through the Social Security Administration (SSA). Members who pay their own Part B premium are eligible for the Giveback. The monthly credit is applied on either the member's Social Security check or Medicare Part B statement, depending on how they pay their Part B premium. It can take a few months for this Giveback to be processed, so the member may receive it as a lump sum.
²This is a general copay that applies to specialist visits for consultation, diagnosis, and treatment by a specialist in a physician’s office, but copay amounts vary for some specialist visits including podiatry, psychiatry, pulmonology, cardiology, etc. Please reference the plan's Evidence of Coverage for a complete listing of benefits.
³OTC balance does not roll over to the next quarter. Can be redeemed online or at participating local retailers.
4The medical, dental, vision, and hearing flex benefit allowance preloaded on the IBX Care Card is a separate wallet from the OTC benefit provided on the same card. No activation is required. Any unused balance will not roll over to the next year. Members should retain the card through the expiration date.
