Understanding your network
You have access to a large provider network of doctors, hospitals, and health care facilities in our area.
Keystone 65 HMO members
As an HMO member, your health plan covers care received from in-network providers. However, out-of-network emergency care is covered for medically necessary services for emergency care. Keystone 65 Focus HMO-POS members have an annual point-of-service maximum that can be used towards out-of-network care, but they'll pay less for in-network care. Please note that the Keystone 65 Focus plan uses a defined network that is a subset of our Keystone 65 HMO network.
You are required to receive in-network laboratory services (like blood work) at a site your Primary Care Provider (PCP) chooses. You can view your PCP's designated site on the front of your member ID card or by logging into ibx.com.
Personal Choice 65 PPO
As a Personal Choice 65 PPO member, you have the freedom to choose any doctor or hospital in- or out-of-network. But you'll pay less by selecting a doctor or hospital in the network.
Dental, vision, and hearing
Staying healthy includes taking care of your oral health, vision, and hearing. That’s why all of our 2025 Medicare Advantage plans offer robust dental, vision, and hearing benefits. There are NO copays for routine in-network dental, vision, and hearing exams; hearing aid fittings and evaluations; and more. And other in-network dental, vision, and hearing services are much more affordable, too.
Regular dental care not only keeps your smile intact and your mouth healthy; it also helps catch other potential health problems before they become more serious. Taking care of your vision can help prevent falls and maintaining good hearing can reduce your risk of social isolation and dementia.
IBX Medicare members receive access to benefits such as:
- $0 copay for one in-network oral exam and teeth cleaning every six months and — new for 2025! available on most plans — one fluoride treatment and two dental consultations every 12 months
- Annual allowance for comprehensive dental services, including implants, fillings, root canals, crowns, partial bridges, dentures, anesthesia during oral surgery, and more — available on most plans
- $0 copay for one in-network routine eye exam every year
- One pair of eyeglasses (frames and lenses) or contact lenses covered per year:
- Eyeglasses are covered in full when purchased from the Davis Vision Collection
- $250 allowance to pay for eyeglasses when purchased from Visionworks®1
- $150 allowance to pay for eyeglasses when purchased from a Davis Vision provider
- Or a $150 allowance every year for contact lenses (instead of eyeglasses)
- $0 copay for one in-network routine hearing exam every year
- Unlimited in-network hearing aid fittings and evaluations for the first year — at no cost through TruHearing

Primary care provider
Primary care is an important part of staying healthy— and a primary care provider (PCP) can be your main partner in doing just that! A PCP will get to know your health history and medical issues, and provide basic medical care, including annual health checkups. They will also answer your health-related questions and help coordinate services with specialists and other providers (no referrals needed).

For Keystone 65 HMO Members: You are required to choose an in-network PCP. No matter which in-network PCP you choose, your copay will be $0, and you won't need a referral to see a specialist. However, you must see specialists within our network.
Keystone 65 HMO members must notify us once you choose your PCP.
To choose your PCP online, log in or register at ibx.com/login. Click on the My Care tab and select Choose my Primary Care Physician from the drop-down menu.

For Personal Choice 65 PPO Members: Your plan doesn’t require you to choose a PCP, but it’s still a good idea as they will help coordinate your care. Whether you choose a PCP or not, you don't need a referral to visit a specialist. You may see health care providers whether they’re in or out of our network, but you’ll usually pay less by seeing in-network providers.

Care in your home
Receive care right where you’re most comfortable, your home. Below are home-based primary care and virtual care options that are available through your IBX plan.
Primary care
Good primary care is fundamental to your health and well-being, but sometimes getting to a doctor’s office for a visit can be a challenge. That’s where Patina can help. They offer comprehensive primary care that comes right to you and is just for adults 65+.
As experts in this phase of life, Patina takes the time to know you and create a care plan tailored to you. The best part is that their care comes right to you, in the comfort and convenience of home or wherever you are, through both virtual and in-home visits. Whether you’re active and healthy or managing more complex conditions, Patina is there when you need them (including 24/7 if care is needed after normal business hours).
- A dedicated, physician-led care team: Your team includes a primary care practitioner, a health champion, and a nurse champion, who deliver high-quality care and help you with legwork.
- Support for your total health and well-being: Patina supports all your needs: physical, emotional, and social.

Patina primary care is available at no extra cost beyond your standard in-network primary care physician copay.

Telemedicine
It’s not always easy or convenient to go to a health appointment. With Teladoc Health, you get quality care for non-emergent general medical, mental health, and dermatology conditions. Connect virtually from your home or when traveling via your computer, tablet, or smartphone, at no cost.
- General medical care: You have 24/7 access to board-certified doctors who can diagnose and treat non-emergency conditions such as sinus infections, flu, sore throats, allergies, earaches, pink eye, and more.
- Mental health care: Schedule visits with a licensed therapist, psychologist, or psychiatrist and get support for depression, anxiety, stress, grief, and more.
- Dermatology: Connect online with a board-certified dermatologist who can provide timely answers to questions about new or chronic skin conditions like rashes, acne, eczema, and rosacea.
Visits are available in several languages through an interpreter, including American Sign Language (ASL).

Preventive care
Many health conditions are easier and less expensive to manage when they’re detected early. That’s why visiting your PCP and prioritizing preventive care is so important!
An annual wellness visit with your PCP is available at no cost — so schedule yours today! During this visit, you can:
- Review your medical and social history
- Identify any risk factors you might have for various health conditions
- Learn about recommended preventive screenings, tests, and vaccines
IBX Medicare Advantage plans pay for a yearly checkup with your PCP. They also pay for many preventive screenings at no cost to you, including tests for:
- Breast cancer
- Bone mass
- Colorectal cancer
- Diabetes
- Eye problems
- Kidney disease
- High blood pressure
You can also get common shots to protect you from diseases at no cost. (Note: Most shots covered by Medicare Part B and D are free for you.)

IBX Care Card
Your IBX Care Card is a convenient way to save money and access your over-the-counter (OTC) benefit and, for specific plans, a dental, vision, and hearing (DVH) flex benefit or a Medical + DVH flex benefit. Your IBX Care Card is separate from your member ID card and comes activated — simply swipe your card and select credit — no pin is needed!
Quarterly over-the-counter (OTC) benefit allowance
Available to all individual IBX plans, your OTC benefit includes a quarterly allowance preloaded on your IBX Care Card to pay for eligible OTC health and wellness items at participating stores. Use your allowance to buy things like cold and allergy medications, first aid supplies, blood pressure cuffs, and more. View our top 30 OTC products available for purchase.

- In-store: Visit mybenefitscenter.com, use the OTC Network® App, or call the Member Help Team for a full list of participating stores and eligible products.
- Online: Shop online at conveybenefits.com/ibx. View the Convey Catalog for a list of available items.
- By phone: Call 1-855-885-5740 (TTY/TDD: 711) to place your order with a Convey OTC specialist, Monday through Friday, 8 a.m. to 11 p.m.
Your OTC allowance is provided quarterly (every three months) and does not roll over to the next quarter if it is not used.

Annual dental, vision, and hearing (DVH) flex benefit allowance
Exclusively for Personal Choice 65 Elite PPO and Personal Choice 65 Prime PPO plan members
In addition to the OTC benefit, Personal Choice 65 Elite PPO and Personal Choice 65 Prime PPO plan members will receive an annual $300 DVH allowance preloaded on your IBX Care Card to pay for out-of-pocket dental, vision, and hearing expenses. Services and products must be provided by a licensed dental, vision, or hearing provider that accepts the IBX Care Card.
Your DVH allowance does not roll over to the next benefit year.

In addition to the OTC benefit, Keystone 65 Basic HMO and Keystone 65 Essential HMO-POS plan members will receive an annual $300 Medical + DVH allowance preloaded on their IBX Care Card to pay for out-of-pocket medical, dental, vision, and hearing expenses. Services and products must be provided by a licensed medical, dental, vision, or hearing provider that accepts the IBX Care Card. Stay active with a free fitness membership through One Pass®!
A One Pass membership gives you access to a variety of physical, social, and mental fitness programs. You can visit one of the many gyms in the One Pass network (including all local YMCAs), take in-person or virtual classes, attend social events and classes, receive a home kit shipped right to your door, get access to brain workouts, and more!

Personal health visits
IBX has contracted an independent third-party vendor to offer you a personal health visit at no cost. A personal health visit can help you stay up to date with your health care in between your regular doctor's visits. It is meant to support your overall health in addition to, not as a substitute for, any other doctor’s appointments you have scheduled.
This in-person or virtual appointment is with a licensed medical professional and will last about 45 minutes to one hour. The medical professional will perform a detailed physical exam, review and discuss your health history, and address any questions or concerns you have. They will always share your exam results with your regular doctor.
As a bonus for completing your personal health visit, we are offering you a $50 retail gift card! Participation in this program is not mandatory, and the visit will not affect your health benefits or premiums.

As an IBX Medicare Advantage member, you have access to behavioral health services and resources. Our trained Behavioral Health Care Navigators are here to help you:
- Understand your needs and provide information about treatment options
- Find in-network care and connect you with a provider quickly
- Offer support during difficult times
- Connect you to the right resources, to ensure the coordination of your behavioral health needs

Registered Nurse Health Coaches and Case Managers
Set, achieve, and maintain your health goals with an Independence Registered Nurse Health Coach available at no additional cost! Members have 24/7/365 access to speak with a Registered Nurse Health Coach.
Health Coaches and Case Managers are specialized registered nurses and licensed social workers who will work with you to:
- Answer your health-related questions
- Assist you in managing chronic health condition(s)
- Identify emerging health risks
- Follow your doctor’s treatment plans
- Navigate your health journey
- Access support for stress, depression, or anxiety
Your Health Coach or Case Manager will provide you with an added level of support to help you stay healthy. Health Coaches and Case Managers may also refer members to an Independence Social Worker for assistance with community resources.

Enhanced Disease Management services
Health Coach services provide extra help to eligible members with chronic health conditions. You may be referred to a Health Coach by your treating physician, or a Health Coach may call you to assess your overall health and well-being.
You have the right to opt in or opt out of these services. Simply advise your Health Coach or Case Manager at any time if you wish to end your relationship with them. You may also call the number on the back of your member ID card and ask that you not be contacted by a Health Coach in the future.

Transportation to health visits
For eligible members of select plans, our transportation benefit provided by Roundtrip makes it easier for you to get to and from your essential health visits at no cost.

Grocery
Provided through FareRx, eligible members can get groceries delivered right to their door at no cost. Your grocery delivery will contain fresh, local groceries that are organic whenever possible, along with recipes for easy and tasty ideas for healthy meals.
For Keystone 65 Basic, Keystone 65 Focus, Keystone 65 Preferred, Keystone 65 Select, Personal Choice Elite, Personal Choice Prime, and Personal Choice PPO plans:
Members who have been diagnosed with both diabetes and a depressive disorder are eligible to get groceries delivered for a maximum of four weeks per year, per member.
New for 2025! Keystone 65 Essential HMO-POS:
Members who qualify for the Medicare’s Low-Income Subsidy (“Extra Help”) program and have a diagnosis of one of the following conditions, are eligible to get groceries delivered for a maximum of 12 weeks per year, per member. Qualifying health conditions include: chronic and disabling mental health conditions, hypertension, diabetes, obesity, chronic kidney disease (CKD), or chronic heart failure (including ischemic heart disease, hyperlipidemia, and peripheral vascular disease).

Visitor/traveler coverage
Personal Choice 65 PPO members are eligible for the optional Visitor/Traveler benefit. You can take extended visits (up to 12 months) to places in the shared Blue Cross®/Blue Shield® visitor/traveler service area without losing your coverage.
The service area includes 48 participating states and two territories2, Puerto Rico and the District of Columbia. You will pay the in-network cost-sharing level for covered benefits while visiting participating providers in the service area.
